How do I file an insurance claim for an injury?

- These are the forms to be used to file a claim with Health Special Risk (HSR) when an injury occurs during an approved Rodeo or Activity. GET FORM HERE

-You will need the signature of the Rodeo Secretary or the National Director on the bottom of the front page to verify that the member was a participant of the rodeo or activity.

- Please read all pages of this form and file all appropriate paperwork timely.

-If you have filed a claim and are having any problems, there is a number on the first page as well as a Customer Service number on the 3rd page to call with questions. If you are still having issues after using these avenues, please call Dixie Huxtable at 307.351.1327, leave a message and she'll look into it.

Do I have to upload the documents, I can't figure out how?

EVERYONE has to upload their documents. There is no other way. You cannot email them to me, you cannot mail them to me. . .THEY HAVE TO BE UPLOADED.

When is the deadline to sign up?

The deadline to sign up is the Friday before you want to enter.

Do the forms have to be notarized?

Yes!

Do all of the forms have to be totally complete in order to become a member?

Where do I pay my fees?

You pay your rodeo entry fees at the rodeo.

Will just my report card or a screen shot of power school work for my grades?

No, for High School you must have a formal transcript. CLICK HERE FOR MORE INFO

What if I don't have a partner for TR or RR?

You have to enter with a partner. The board voted to no longer allow an athlete to draw a partner. I suggest that you put it on our Facebook page.

Can I enter late? What if my paper work is late?

NO late entries or paperwork is accepted.

I did not receive my receipt when I entered

1. Check Your Junk Folder

2. Go back and try to enter again if you can't then you are entered

3. Call in Tuesday morning 6-7 before that rodeo to confirm

The secretary can not see who is entered in the rodeo or make any changes until the rodeo closes at midnight Monday and she downloads it
